6.04 Dollar Contribution Type – Dollar Amount Paid by Employer

  1. Once you select the dollar amount, the following will be displayed:
    1. Carrier Name – From the selected package.
    2. “Flat Monthly Amount Paid By” selections
      1. Employer (default selection)
      2.  Employee
    3. “For Employee” Dollar Amount entry
    4. The “Roll Excess to Dependent” applies any remaining amount after the employee’s premium to the dependent’s premium.
    5. “For Dependent” Dollar Amount entry
    6. “Based on any plan” – The contribution applies to any plan within the package.
  2. Complete all selections/entries.
  3. Discard Unsaved Changes/Save Changes
    1. Click the Save button to save the data.
    2. Click the Discard Unsaved Changes to reset all and not save.
    3. If changes have not been discarded or saved, a warning message will appear when navigating away from the screen.
    4. Click Discard Unsaved Changes or Save.
