- When selecting the “Flat Monthly Amount Paid By” Employee, the following appears:
- “For Employee” Text and Dollar Amount Entry
- “For Dependent” Text and Dollar Amount Entry
- Employee pays 100% for dependents – If selected, the employee covers the entire premium for their dependents. The “For Dependent” is set to zero.
- “Based on selected plan” – The contribution is applied to the plan chosen from the list of plans in the package displayed after selecting “Selected Plan”.
- Complete all selections/entries.
- Discard Unsaved Changes/Save Changes
